  • Medical Treatment Authorization

    I understand that the staff at Wylie Elite are not licensed physicians or medical practitioners. In the event of an injury or illness, I authorize Wylie Elite staff to administer basic first aid and, if deemed necessary, to summon emergency medical services, including calling an ambulance. I understand and agree that I am solely responsible for all medical expenses incurred, including emergency transportation and treatment. As the parent or legal guardian, I assume full responsibility for providing health insurance coverage for my child and/or payment of any medical expenses resulting from participation in any Wylie Elite activities.

  • Medical Conditions Disclosure

    Does your child have any medical conditions we should be aware of? (Examples include but are not limited to: asthma, seizures, Down Syndrome, dizzy spells, prior neck or spine injuries, broken bones, high blood pressure, diabetes, autism, epilepsy, heart conditions, etc.) All disclosed medical conditions require a physician’s written release confirming the participant is physically fit to engage in cheerleading and tumbling activities.
  • If my child uses an inhaler, it should be brought to class. I understand I am required to stay with him/her or get doctor’s release.

  • Acknowledgement and Assumption of Risk

  • I consent to my child’s participation in all programs and activities offered by Wylie Elite. These activities include, but are not limited to: gymnastics, tumbling, cheerleading, trampoline use, private lessons, open gyms, birthday parties, virtual training, inflatables, camps, and special events. I understand that these activities are inherently dangerous and involve risks that may include, but are not limited to: muscle or soft tissue injuries (strains, sprains, or tears), broken bones, head injuries, paralysis, permanent disability, or death. These risks may arise from, but are not limited to, physical exertion, use of equipment, height, body rotation, environmental conditions, or actions of other participants. I knowingly and voluntarily assume all such risks on behalf of my child.

  • Release of Liability

    In consideration for Wylie Elite allowing my child to participate in its programs and activities, I, on behalf of myself, my child, our heirs, assigns, legal representatives, and next of kin, voluntarily waive, release, and discharge Wylie Elite, The Studio For Kids, Inc., all owners, officers, employees, agents, contractors, and volunteers from any and all claims or liabilities of any kind, whether known or unknown, foreseen or unforeseen, arising out of or related to my child’s participation. This release includes claims arising from the NEGLIGENCE of the released parties, whether such negligence is passive or active, to the fullest extent permitted by law. I understand that by signing this agreement, I am giving up substantial legal rights.

  • Indemnification and Hold Harmless Agreement

    I agree to indemnify, defend, and hold harmless Wylie Elite, The Studio For Kids, Inc., Jonathan Thain, and their owners, officers, employees, agents, contractors, and volunteers from any and all claims, demands, damages, losses, liabilities, costs, or expenses (including attorney’s fees) arising out of or related to my child’s participation, including claims brought by third parties.

  • Photo and Video Release

    I grant Wylie Elite permission to photograph, video record, and/or otherwise capture my child’s likeness during participation in activities. I understand that such images or recordings may be used for promotional, marketing, social media, educational, or training purposes without compensation and without additional consent.

  • Supervision Acknowledgment

    I understand that Wylie Elite does not provide constant one-on-one supervision and that participation requires a level of physical ability, awareness, and personal responsibility appropriate to the activity. I acknowledge that reasonable supervision does not eliminate all risks.

  • Governing Law and Venue

    This agreement shall be governed by and construed in accordance with the laws of the State of Texas, and any legal action arising from this agreement shall be brought exclusively in Collin County, Texas.

  • Electronic Signature Consent

    I understand and agree that this waiver may be executed electronically and that an electronic signature shall have the same legal force and effect as an original handwritten signature.

  • Severability

    If any provision of this agreement is held to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
  • I acknowledge that I have read and fully understand this agreement. I am signing it voluntarily and certify that I am at least 18 years of age.
